Yes, I understand that by enrolling in the Simply Family Membership, I agree to the following:

Membership is $69 per month, per child. Membership fees will be invoiced separately.
Each membership is assigned to one enrolled child. Any parent, guardian, or approved adult may accompany and supervise the enrolled child.
Membership will continue month-to-month until canceled.
A 30-day written notice is required to cancel membership.
Membership includes unlimited open play during regular business hours for the enrolled child, subject to Simply Play’s posted hours, capacity, closures, and facility rules.
Members receive discounts on select Simply Play events and activities.
Memberships are non-transferable between children.
All children and accompanying adults must follow Simply Play’s current waiver, play, and safety rules.
